This study aimed to develop an automated examination checker, item analyzer and test bank system. Specifically, the study aimed to create a system that could manage test items, generate questionnaires and produce Optical Mark Recognition (OMR) ready answer sheets, check the answer sheet image files using the software-based OMR technology and perform items analysis and recommend items to be retained, revised and rejected and store for data banking. This study utilized two main algorithms in the implementation of software or image-based OMR technology; the projection profile and the skewing algorithms which were used in the reading and identifying the shaded marks or areas in the answer sheet image file. While, the rule-based algorithm was used in the item analysis function of the system to determine the items to be retained, revised and rejected. The researcher used the Modified Waterfall Model as a software development model that serves as guide in developing the system and the Unified Modeling Language (UML) as the software development tool in the design of the system’s structures. To evaluate the system, the internallyrecognized ISO/EIC 9126 Software Quality Standard was employed. The mean and t-test were the statistical tools utilized to analyze the data gathered from the respondents who include: eleven (11) teachers and six (6) IT experts who are systems/network administrators and software developers/programmers. The result of the evaluation showed that the system is “very effective” in terms of its functionality, usability, reliability, efficiency, maintainability and portability – the six (6) Software Quality Characteristics of ISO/EIC 9126.en
